• Medicare-related information, including plan details, enrollment status, and coverage questions
• Healthcare provider, prescription, and coverage information you share so we can assist you

You may withdraw marketing consent at any time. Withdrawing marketing consent does not affect service-related communications about an appointment, enrollment, class, webinar, or other service you requested.

How We Share Your Information

Bellman + Associates does not sell or rent your personal information.

We may share information in the following limited situations:

• Insurance carriers. We may share information with insurance carriers to provide services you request, obtain quotes, or complete an enrollment you have authorized.
• Government agencies. We may share information with the Centers for Medicare & Medicaid Services (CMS) or other government agencies as required for enrollment, oversight, reporting, or regulatory compliance.
• Service providers. We may share information with trusted third-party service providers that help us operate our business and website. These may include our website and form-processing platform, scheduling tools, email and text-message services, website-hosting providers, analytics providers, and other technology providers. These providers receive information reasonably necessary to perform their functions and are expected to protect it.
• Legal and regulatory matters. We may share information when required by law, subpoena, regulatory request, or legal process, or when reasonably necessary to protect our legal rights, prevent harm, or prevent fraud.
• Business transitions. If our business or assets are transferred, merged, or sold, personal information may be transferred as part of that transaction, subject to applicable law and this Privacy Notice.

Our website and form-processing platform, Marblism, processes information submitted through website forms and relays those submissions to us. Marblism may process information on our behalf as a technology and service provider.

Information submitted through a third-party platform is also subject to that platform's own privacy practices and terms. We encourage you to review the privacy notices and terms of any third-party service you use.

Cookies and Analytics

Our website uses behavioral analytics to understand general site usage and improve our content. We do not use advertising pixels, and we do not sell or rent your personal information.

The website platform uses PostHog, a behavioral analytics tool, to record general website-usage information such as pages viewed and navigation patterns. The analytics tool sets a small identifier in a visitor's browser when the visitor visits the website.

The analytics tool operates through the website platform and is used to understand general site usage and improve our content. The website platform may process analytics information on our behalf.

We do not use advertising or tracking pixels. The website does not use Google Analytics, Google Tag Manager, Meta or Facebook Pixel, LinkedIn Insight Tag, TikTok Pixel, Pinterest Tag, or Google Ads tracking. The website also does not use heatmapping or session-recording tools.

Our scheduling links are ordinary links. If you click a scheduling link, you leave our website and go to our scheduling provider, Calendly. Calendly has its own privacy practices and terms. No Calendly scheduling cookies are set on our website, although Calendly may collect information when you use its scheduling service.

Our website does not currently present a cookie consent banner. Accordingly, the analytics identifier is set when a visitor visits the website.

Most web browsers allow you to block or delete cookies and site storage through your browser settings. Blocking or deleting cookies or site storage may affect certain website functionality.

Data Retention

We retain personal information only as long as reasonably necessary for the purposes described in this Privacy Notice, plus any period required by law or by applicable insurance carrier, CMS, tax, and regulatory obligations.

Medicare enrollment, client, and beneficiary records: We retain these records for a minimum of ten (10) years from the date of the last transaction or as otherwise required by CMS, an insurance carrier, or applicable regulatory requirements.

Marketing and permission-to-contact records: We retain these records as necessary to honor communication preferences and document consent, and longer where required by applicable law.

General inquiries and appointment records: We retain these records for a minimum of ten (10) years or as otherwise required by applicable regulatory requirements.

Financial and tax records: We retain these records for a minimum of ten (10) years or as otherwise required by applicable tax and regulatory requirements.

Where a specific record is subject to a longer retention period under CMS, insurance-carrier, or other regulatory requirements, the longer period controls.

When information is no longer needed and no retention obligation applies, we take reasonable steps to securely delete, destroy, or de-identify the information.

How We Protect Your Information

We use reasonable administrative, technical, and physical safeguards to protect personal information. These safeguards include limiting internal access to individuals who genuinely need the information, using reputable service providers, and requiring appropriate protections where possible.

No method of transmission or storage is completely secure. We cannot guarantee the absolute security of personal information.

If we become aware of a security incident affecting personal information, we will notify affected individuals and applicable authorities as required by applicable law.

Bellman + Associates is based in Ohio.

Some of our service providers may store or process information on servers located outside Ohio or outside the United States. Where information is processed across state or international borders, we rely on our service providers to apply appropriate protections and safeguard information consistently with this Privacy Notice and applicable law.

We may update this Privacy Notice periodically to reflect changes in our practices, technology, or applicable requirements.
